Frey syndrome is due to the misdirected regenerative process of the auriculotemporal nerve. This nerve is a sensory branch of the posterior division of the mandibular division of the trigeminal nerve. The sensory fibers innervate the auricle, external auditory meatus, tympanic membrane, temporomandibular joint, and the temporal scalp.
